Pastoral Care of the Sick

When a parishioner is confined to home because of age or infirmity complete the form below to arrange Holy Communion at home. When a parishioner is in a hospital or nursing home please complete the form below to arrange for a visit . When one is seriously ill, the healing Presence of the Lord should be requested in the Sacrament of the Sick by calling the Parish office 419-472-2288.
